You will need to bring: 

• Proof of past 12-month income (ie.: 2023 tax return, W2, paystubs, TANF/SSI verification, child support, SNAP food stamps)
• Child’s Birth Certificate
• Medical Insurance Card
• Shot/Immunization Record

Also, the items below if they apply to you:

• Proof of Dr. documented disability/mental health 
• IEP
• Custody Papers
